Community Based Services
Providing Healing and Hope Wherever You Need it Most
When you think about receiving services, where do you see yourself? At the park? In a library? On a walk? How about in your own home? These places offer comfort to so many, and our team can meet you there. That’s right, in a place where you are most comfortable. Our Community Based programs provide services in Dutchess, Orange, Putnam, Sullivan and Ulster counties. We strive to provide the highest level of care – care that is centered around your child and accessible to your family.
Allowing these services to take place where youth are most comfortable creates the strong rapport that is necessary to facilitate hope and change.
One vision, one team. Allow our Care Manager to help coordinate all support services for youth. Many times it can feel overwhelming when support providers work in silos and it doesn’t seem that they know what the other is telling your family. Our Care Manager can bring everyone together with a comprehensive plan to accomplish goals set by your family.
Our Children and Family Treatment Support Services include individual, family and group counseling. This program has components of targeted interventions to achieve optimal health and well being for youth and their families along with skill based components such as prevocational and daily living skills. The youth voice is so vital in the work we do and our team wants to be sure the youth and family feel empowered.
Home and Community Based Services provides four specific services to youth including respite, provocation skills, and caregiver/family support and advocacy. Please note to be eligible for HCBS the youth must first be enrolled in Health Home Care Management.
Wendy’s Wonderful Kids is a program funded by a grant from the Dave Thomas Foundation for Adoption, a national non-profit public charity that brings children waiting in foster care one-step closer to being adopted into safe, loving and permanent homes. Our WWK program provides child-focused adoption recruiters that work in Dutchess, Ulster and Sullivan counties in New York.
