    Care Management Services

    The Children’s Home provides Care Management Services as part of the New York State Health Home Care Management Program. Children from birth to age 21, who are Medicaid eligible, receive assistance from a Care Manager to help manage the child’s chronic and complex medical or behavioral health condition. The Children’s Home assigns a Care Manager to provide services to families and youth. Care Management provide the tools necessary to help families stay healthy, promote wellness, and decrease utilization of the hospital based services. The Care Manager provides a family centered plan that supports continuity of care and will help families arrange new services if necessary.

    About Children’s Health Home of Upstate New York

    CHHUNY (Children’s Health Home of Upstate New York, LLC) is an independent entity created by twenty of Upstate New York’s leading child and family agencies to establish a Children’s Health Home. Over 120 local agencies from Western New York to the Hudson Valley have joined CHHUNY’s network. CHHUNY uses a model of care management based upon the belief that every family has strengths and that families are the natural and most effective drivers of care.

    Who is eligible to enroll?

    Children from birth to age 21 must have insurance and have two or more chronic health conditions OR one single qualifying condition:

    • Children from birth to age 21
    • Must have Medicaid or be Medicaid eligible*
    • Two or more chronic health conditions OR one single qualifying condition:
      • HIV/AIDS
      • Severe Emotional Disturbance

    *The Children’s Home of Poughkeepsie has experts on staff that will be happy to work with you to determine if your child is eligible.

    What does a Children’s Health Home do?

    Once you enroll in a Health Home, a Care Manager will work with you to develop a plan of care for your child. Specific services may include:

    • Patient Advocacy
    • Development of a Family-Centered Plan of Care
    • Medical Transportation Support
    • Health Promotion
    • Wellness Education
    • Referral to Community and Social Support Services
    • Crisis Management
    • Individual and Family Support

    Advantages of the CHHUNY Network

    Families have the choice to select from a number of approved Children’s Health Homes that are authorized by the New York State Department of Health. The CHHUNY network, of which the Children’s Home of Poughkeepsie is a member, is New York State’s largest and serves 54 of the 61 counties in New York State. This large network not only gives families access to the largest list of providers and Care Managers, but the services can travel with you.
