Care Management Services
The Children’s Home provides Care Management Services as part of the NYS Health Home Care Management Program. Children from birth to age 21, who are medicaid eligible receive assistance from a Care Manager to help manage the child’s chronic and complex medical or behavioral health condition. The Children’s Home assigns a Care Manager to provide services to families and youth. The goal of these services are to ensure they have everything necessary to stay healthy, promote wellness, and decrease utilization of the hospital. The Care Manager provides a family centered plan that supports continuity of care and will help families arrange new services if necessary.
